We schedule the patient
Our team contacts the patient directly, confirms the nearest location with availability and books the initial evaluation, usually within a few business days.
You receive documentation
After each visit we share the evaluation, treatment plan and progress notes with the referring office or firm so your file stays current.

Referrals are accepted by email or phone. We review correspondence regularly, including evenings and weekends, so referral partners get quick answers.
What to include
- Patient name, phone number and email
- Date of loss and mechanism of injury
- Body parts involved and current symptoms
- Imaging and prior treatment records, if available
- Attorney or case manager contact, for personal injury cases
- Preferred location: Fort Myers, Hollywood or Kendall/Miami
