We use the information you submit to respond to your request, schedule appointments, coordinate care with referring providers, and send you information you have asked to receive.

We do not sell your personal information. We do not use information submitted through this website for advertising to third parties.

Protected health information

Any protected health information you share with us in the course of care is handled under HIPAA and our Notice of Privacy Practices, which governs in the event of any conflict with this policy.

Please do not submit detailed medical records or urgent clinical information through the website forms. Call our office instead.

Sharing

We share information only with people and organizations involved in your care or request: our clinical and scheduling staff, referring providers, and, where you have authorized it, the attorney or case manager handling a personal injury claim.

We may also disclose information where required by law.

Your choices

You may ask us to correct or delete the contact information you submitted, or to stop sending you email, at any time.
